Conditional logic does not work

    Now I'm making my new forms. However, I faced a problem.
    At the time I add a field product using conditional logic, all of a sudden my form does not appear on the page preview. But, when I remove the conditional logic of my form back to work. I desperately need an answer soon!
    Sorry with my english is messed up. Any help given is appreciated. Thank you.

    Have you tested for theme/plugin conflicts yet?

    As I said, before walking fine (maybe about 1 month), meaning he does not have a conflict with my theme. So is the plugin I've disabled all plugins and re-test the Grafity forms but still, he does not work
