Conditional Logic - Form not showing up

    Hi GF,

    I am trying to set up a very basic conditional logic where if you select a job title and location, it shows a specific html block with a link in it.

    I've set it up very basic and once I enable conditional logic, the whole form does not show up. Please note there is no conditional logic enable on the location or job title.

    Have you tested for a theme or plugin conflict yet?
