Conditional Logic not showing designated fields.

I've purchased the pro version and am having issues with the conditional logic function. I've got a drop down with 5 choices. the 1st 2 choices will bring up a new drop-down (the same drop down). The last 3 choices bring up a description box.

For some reason, when i select any of the drop-down's choices, neither the next drop-down or description box appear! Any ideas? I need this for a client ASAP!! It's due tonight/tomorrow.

Transfered the site to the clients server. Thought maybe that would fix the issue. The fields still don't appear when i make a selection from the drop-down.

Here's a link for anyone that even feels the slight bit helping me to resolve this. When you make a selection on the "I am a:" drop-down in the form, either a new drop-down or text box should appear.
