    need to add a Contact Me button below a name that will show a quick pop-up form..

    What have you tried so far? Creating the button is not related to Gravity Forms. The popup functionality (lightbox?) is not provided by Gravity Forms. The only portion of this which is related to Gravity Forms is the form itself. Have you already created that?
