Create a draft post -- proper newbie, not a clue

Hi, I'll try and make this simple. I'm a massive newbie, and have no idea what I'm doing.

I'd like to create a form with three single line text boxes -- "name", "age", and "location" and one paragraph text box called "message".

The content of these three text boxes would then come together to formulate the draft post title, like "Name - Age - Location", with the paragraph text box being the body of the post. Once the user hits submit, in my entries folder will be a post sitting in draft, for me to then moderate and hit publish.

Sounds easy enough in theory -- but I can't figure out for the life of me how to do this. I've tried searching but nothing makes sense. I'm not a programmer, at all.

If anyone can walk me through this, I'd be most thankful.
