Date Picker Not Working

    The date picker isn't working on this form: http://www.sawyersrun.org/registration/
    From the forums I've checked these other things to no avail:
    Disabled all plugins and still broken
    Enabled WP 2011 theme and still broken
    Has wp_head and wp_footer
    Ajax is enabled
    Works in the preview

    What have I missed checking? Any suggestion what to try next?

    Can you disable the Mailchimp plugin?
