Default Op-in Form showing up on page when it is disabled

    I am using the Headway Theme. On one of my pages, I have my the gravity form opt-in disabled, and it is showing up anyhow. In fact, it is showing a totally unformatted default option. How can I get it to go away?

    And if I actually choose to have it put in the opt-in that I designed, it shows both that and the default one. What to do?

    We don't have a default opt-in form. Maybe Headway includes one? Can you point out what form you mean?
