I took the advice and added "gf_list_3col" to the CSS Style area in advanced for the donation portion of the form. It is still lining up vertically.
Do the styles work for the CSS Styling?

It doesn't appear that you have added the gf_list_3col custom class to your CSS Class in the advanced tab of that donation field and saved your form. When I view source on your form, I don't see the gf_list_3col class applied to that field. See this screenshot: http://grab.by/a5ma

When I view source on my test form I see this: http://grab.by/a5mq
Notice in my example the gf_list_3col class is present when viewing source and in yours it is not there which is why it isn't being applied.
