I have tried deactivating plugins and can't seem to figure out why it is suddenly happening.
Any help would be greatly appreciated.
This is going to be a theme or plugin conflict. Something else you are running is interfering with Gravity Forms and outputting code on that page. Often this happens because a theme is outputting Javascript on EVERY page, including admin pages when it shouldn't be.

You would have to discuss this with the theme developer, is it a commercial theme that includes support or is it a free theme?
The problem is the theme is outputting Javascript and CSS on EVERY page (including the Gravity Forms related admin pages) which is causing the conflict. This happens when theme developers don't follow best practices and only output scripts and CSS when necessary.
It is actually a paid theme from templatic. I will contact them now.
