embeding a form

  1. I've used the add gravity form icon to embeded a form within a page. Here's the code and what was embeded on the page: [gravityform id="1" name="Test Form" title="false" description="false"]. I can view the page but the form does not appear.

    I've been able to view forms on this page with formbuilder and contact form 7. Can someone tell me why I can't view the gravity form?

    It finally started to appear on the page. Here is the link to the page.

    Can you tell me how to put the checkboxes in 2 columns, not just one vertical column.
    Also, can you tell me why the address fields have no seperation between them and why the addresss labels appear to the right of the blank field/box?

  4. It looks like our stylesheet isn't getting output on the page. Do you have our CSS set to output on the Gravity Form's Settings page?

    Once you do that, the styling should clear up and then you can use our ready classes for the checkbox question you had:

  5. Rob, thanks for your assistance. I'm new at this so go easy. I've checked the Gravity Form's settings page and here what I've got. Both the output CSS and output HTML5 are set to "Yes". No-conflict mode is set to "off". Is this what you're talking about?

  6. Hmm - so it sounds like your theme is preventing the stylesheet from coming through. Could you send me a WP Admin login to rob@rocketgenius.com and reference this email so I can see if there are any suggestions/fixes I can offer you.
