error in gravity forms

    i have a form on this page http://estopforeclosure.com/hafa-short-sale-quiz/

    When i insert the gravity form there is the following message in with the form... Notice:Undefined index:month in /home/amuser1/public_html/estopforeclosure.com/public/wp-content/plugins/gravityforms/common.php on line 2148Notice:Undefined index:day in /home/amuser1/public_html/estopforeclosure.com/public/wp-content/plugins/gravityforms/common.php on line 2151Notice:Undefined index:month in /home/amuser1/public_html/estopforeclosure.com/public/wp-content/plugins/gravityforms/common.php on line 2148Notice:Undefined index:day in /home/amuser1/public_html/estopforeclosure.com/public/wp-content/plugins/gravityforms/common.php on line 2151

    Any idea what this is?

    Do you have debug mode turned on in your wp-config file? These look like debug warning notices, not error messages.
