Which version of WordPress and Gravity Forms are you using? Are you using any add-ons from third parties? Can I have you try the following instructions:

Tried switching to twenty ten and the error is still there.

  4. The error is coming from line 3774 of gravity-forms-addong.php in /wpcontent/plugins/gravity-forms-addons.

This error is not coming from Gravity Forms, it's coming from a 3rd party plugin that you are using. The Gravity Forms Directory Add-On. We did not create this add-on and therefore it's not an add-on that we provide support for.

You will need to contact the developer of this plugin regarding this issue.

We did not create this add-on, it is a 3rd party add-on that was created by another developer. You can find the WordPress.org plugin page for this plugin here:

    http://wordpress.org/extend/plugins/gravity-forms-addons/

There is a support forum for this plugin here:
