Error with form

    I am getting this response when I load my form
    Sorry. This form is no longer accepting new submissions.
    I have tried theme change and plugin conflicts but nothing is working. My website is http://www.undergroundwriter.com

    Edit the form and check the advanced tab of the form settings, is limit number of entries ticked, if it is that would prevent the form from accepting new submissions if the limit has been reached. http://www.gravityhelp.com/documentation/page/Form_Settings
