Fields showing from deleted forms.

    I had 3 forms then had an issue and noticed that form fields were showing on form it was not assigned to.

    So I deleted the form.

    Then I still saw the fields.

    So I deactivated and deleted the plugin and then reinstalled it. I still see those fields.

    Do you have a database schema so I can go in and remove those rows?
