Form did not start as scheduled

We had a group of forms scheduled to start at 12:00 AM on May 1, 2013. We double -checked that the settings were correct, and for all intents and purposes, everything looked right.

However, the forms never started as scheduled.

Any thoughts as to what might have gone wrong?

Server time or Database time are not set to the same timezone as your WordPress installation? Caching plugin didn't clear/refresh cache when form was supposed to go live? Those are the two likely ones I can think of.
