Form does not appear in IE9

    My form on this page does not appear at all for me in IE9: http://ckdis.co/RUHS30.

    It works just fine in Chrome, Firefox, and Safari.

    The preview form does appear. I have gone through the steps here to no avail: http://www.gravityhelp.com/documentation/page/Testing_for_a_Theme/Plugin_Conflict.

    I have figured out the issue, so this can be closed. WordPress was adding some formatting around the form shortcode. The problem disappeared after I removed it.
