Form does not display properly

    I am using the following shortcode on http://www.agapeschoolmission.com.php5-22.dfw1-1.websitetestlink.com/contact-us/:

    [gravityform id="1" name="Contact Us" title="false" description="false" ajax="true"]

    Where the form should be displayed, I see jQuery markup instead. If I change the ajax value to false, I see nothing at all.

    Any ideas as to what's causing this?

  2. That's an odd one. Have you tested for theme/plugin conflicts yet? Does the form show up OK in the formbuilder preview?

    Same result when all other plugins are disabled, and the form does work in the preview. The problem also exists in Twenty Ten and Twenty Eleven.

    Also tried re-uploading the plugin files, to no avail.

    I had placed the shortcode inside a div (with blank lines still surrounding it, though), which I'd planned to float next to the embedded map. Removing that div solved the problem, though I have no idea why.
