Form doesn't show up. Shows Price: $

    I created a form and it shows up as this 'Price: $' on the the page. Anybody else have this problem?

    I don't see a Gravity Form embedded on that page. The Price: $ is something that looks like it was manually added. Other than calling a Gravity Forms JS file, it doesn't look like you are using Gravity Forms correctly.
