Form Gone After Wordpress Upgrade to 3.5

    I have had a form up for 3 months and after upgrading to wordpress 3.5 if just disappeared from the contact page. Weird this is my other form on a different page is still working.

    I deleted the code from the contact page and tried to add it in again using the form button. But when I click on the button nothing happens.

    I did not make any changes to the form and did not change anything on the site other then the upgrade this morning.

    Any ideas?

    Same here. Except it's only on one site (http://designpx.com/contact/). All the other sites I've upgraded to 3.5 and have GF on work fine. What's the deal?
