    My form layout is crazy...check it out...

    I tried to use a {raw} tag as well but did not help.....It's worked fine before with this theme http://grab.by/aWfs ...ugh

    Not sure what the raw tag has to do with it. I wouldn't recommend putting the form shortcode inside of the raw shortcode, it can cause a variety of problems.

    That said, the problem is that your plugin folder/path name is wrong. It's looking for a forms.css file in the "gravityforms2" folder ( which doesn't exist ) when it should simply be looking in the "gravityforms" folder.

    Check your installation. I would totally remove any and all instances of the plugin, then do a fresh install. That should get you rolling.
