Form not displaying after update to v1.6.4.1.1 AND v1.6.4.2

I held off on posting before when I learned about the 1.6.4.2 update that was coming, hoping that would fix my forms, but I have implemented the update, flushed my cache, and still am not having any luck.

My forms have not worked since the 1.6.4.1.1 update, and I am getting inquiries from clients every day asking for alternatives to the forms, which I would prefer to not have to offer.

I checked for plugin conflicts, and the culprit is Crowd Favorite's Social Plugin:

I've left it deactivated for now, but any thoughts as to a solution?
