Form Not Found

    I'm receiving an error "Form Not Found", my logs show that gravity form is looking for the following files and it's returning 404 errors:

    gravityforms/css/images/ui-bg_glass_65_ffffff_1x400.png
    gravityforms/css/images/ui-bg_flat_75_ffffff_40x100.png
    gravityforms/css/images/ui-bg_glass_75_dadada_1x400.png
    gravityforms/images/new-form-option-bg.png
    gravityforms/css/images/ui-bg_glass_65_ffffff_1x400.png
    gravityforms/css/images/ui-bg_flat_75_ffffff_40x100.png
    gravityforms/css/images/ui-bg_glass_75_dadada_1x400.png
