Form Title Showing Even When "False"

    I've placed a form on this page: http://sed-efca.org/church-starts/church-starts-contact-page/
    and, even though I selected NOT to show the title, it still shows.

    This is the code in WP:

    As soon as I posted this I realized what my problem was; I had not set "description to 'false'.

    Problem is resolved now, sorry for the trouble.
