Form with conditional logic not showing after update V 1.6.4.1.1

    Hi,

    It seems to be conditional logic. My other form without conditional logic shows up just fine.

    http://www.libertarischepartij.nl/registreren/ (with conditional logic) The form displays in the formbuilder preview but not on the webpage.
    http://www.libertarischepartij.nl/lid-worden/ (without conditional logic) No problems.

    So there is a JavaScript issue here. It looks like it's coming from nmedia-mailchimp-widget. That plugin is including an old version of jQuery (1.4.4). Gravity Forms enqueues and requires the latest version of jQuery that ships with WordPress.
