Form won't submit

    The form simply doesn't submit. Tried in Chrome and Firefox. It is hosted on HostMySite.com. It works perfectly fine on our development URL though. There are no errors, it just reloads the page as if I hit Refresh in the browser. How can I start debugging this?

    It does work when using the Preview version of the form, so how can I tell what is preventing the form from working on the frontend? As mentioned, it works just fine on my development URL - http://ashwebdev.com/womens-imaging/contact
