Form works on one page but not another plus i don't get email notification

    Hello - please see this page:
    http://www.medicalfitnesspros.com/contact
    Then this page:
    http://www.medicalfitnesspros.com/complimentary-consultation
    So, the 'contact' page form doesnt function at all - entries don't show up under entries and I get no email notifications.
    Also, the 'complimentary consultation' page DOES show up under entries yet still, I get no email notifications at all.
    Can you please offer any suggestions??
