Forms gone but still can see some data

    Running: Gravity Forms 1.6

    Page URL: http://writersinstitute.gc.cuny.edu/application/application-form/

    Sometime in the last two weeks, my form has:

    1. gone missing from my site: "Oops! We could not locate your form."
    2. none of the Entries or Form Editing information is available on the Dashboard
    3. Dashboard still says form still exists, and can be selected as an existing form.

    I checked my database and the content is still in there.

    I used the Meta Recovery Plugin but it just told me: "This form's meta could not be recovered".

    Any thoughts on where to go from here would be helpful.

  2. It sounds like something has happened to your database. This only happens when data has been corrupt. Have you recently migrated your site from one server to another? Has your host recently migrated your site from one server to another behind the scenes? Are you aware of any changes that have taken place to your web server?
