the add form button on the post editor doesn't work. It pops up the dark overlay and the loading graphic, but gets stuck.

  2. I am using the 1.6beta2 without any of those issues. The add form functionality works and the trash function works perfectly. Sounds like you have a plugin or script conflict and should start by checking for those.

Also, please try a different browser to be sure there is no cached JavaScript or anything like that.

  4. Good information to share. Thank you for that. I'll install with the latest WordPress 3.3 and see if I can recreate this. Thanks for reporting it.

  5. Thought I'd chime in here that I'm on 3.3 using 1.6RC1 and I'm having the same issues.

  6. This is a compatibility issue with WordPress 3.3 which is not even in Beta yet. Keep that in mind when running bleeding edge versions of WordPress.

The issue is due to changes WordPress 3.3 made to the built in modal window script. We were already aware of the issue but this isn't something that is going to be corrected until WordPress 3.3 is pretty much wrapped up due to the fact anything can change between now and then.

Once it's closer to release, then we will release updates to ensure compatibility with any changes made in core.
