I am able to create and view my form. However when I add it to a page and publish it, I get the code for the form instead of the form itself.

Gravity Forms works just fine with the Default TwentyTen theme. Your theme may have been based on TwentyTen, but it's far from the default now.
Something in your theme.. most likely another plugin is preventing the Gravity Forms shortcode from working.
Try disabling all of your other plugins, see if the form embed works then. If so, then toggle the plugins back on once at a time until you find out what's causing the issue.

Mmkay. Try rolling back to the actual default TwentyTen theme and see if it works there. If so, then it could be something in your theme itself.
Okay, then try uninstalling Gravity Forms completely, then reinstall. Occasionally WordPress flubs the installation process.. this could be one of those instances.
This didn't work either. I have the most current version of gravity forms. So frustrating. I've used the forms on two other sites with no problem. Can you log in and look for me? I just don't know what else to do.

Kevin, I did fill out this form yesterday with my info, but just noticed it says that form is not for support. I do not want my support request ignored so please advise if I need to do something differently. Thanks.

Something is wrong with your WordPress installation. It's not that Gravity Forms isn't working, it's your site isn't processing shortcodes correctly.
I created a test page called Test Page and tested using the video plugin you have installed and inserted it's shortcode that calls a Kaltura video into your site. It didn't work either, it rendered the shortcode.

So something is wrong with your WordPress site itself. I tested with all other plugins deactivated and with another theme, same issue, so it's an issue with your core WordPress files or possibly a server file permission issue. I would suggest re-installing WordPress on this site.
