Gravity Forms not displaying

    Hi

    I have just installed gravity forms and created my first form - however the form does not display on the page that I created. I have deactivated and then reactivated all my plugins to see if there is a conflict but even when I have no other plugins activated the forms do not display. http://elmurphy.co.uk/contact-me-2/
