I can not see more fields to fill

    It's a theme style issue. Line 18 of your theme's stylesheet..

    sets a "border: 0 none" property on text and password inputs as well as textareas. That's why you're not seeing the borders. If you remove that rule, then you'll see that the inputs are displayed normally.
