Issues With Entries

    I am having issues with the entries for my forms. The forms works well, but I am having major issues when viewing the entries in the admin section.

    For example, I am working with schools. The main entries page shows the proper information, but when viewing a particular entry it shows another school name. So the main entry page shows all of the schools, but when trying to view ABC school it shows information for DEC school.

    Any suggestions on how to correct this?
