need a form to populate from page author and then email third parties

    i need to be able to have the "contact this agent" auto populate the agent's email from their page and then send the contact form to multi-parties. i should be able to do this myself, however, not finding exact tutorial. also, need a template page that auto-pops from a contact submission form such as http://www.mountainofagents.com/index.php
