Page Break - Submit button (out of place)

    Sorry for the inconvenience
    Can you tell me how to do, so the submit button, form, appear in the right place.
    This only happens when I use the "Page Break"
    example:
    http://edsoncruz.com.br/2013/orcamento-para-seguros/

    1st option + Name (fill)
    You see the error.

    It is in the code?
    Where can I fix?

    I don't even see a submit button. Can you give me more information please about what you are doing? Do you have any form fields after the page break?

    Making a form.
    You tried to enter the 1st option
    Then put your name
    and see the following page
