Phantom entries and 'required' not respected

Got a weird one here. Almost 300 entries in the last few days with no details in the entries and required fields not filled out. Entries says "Displaying 261 of 281" yet also says "This form has no entries". Here's a screenshot: http://cl.ly/image/1a0Z2q2e4109

Any thoughts?

My first thought would be caching. My second thought would be that it's a script submitting the form. Can you correlate the entries to the web server logs?
