Phone and Fax Number on Form

    When I add 2 phone number fields, I am not able to remane one for Fax and one for Phone.

    When I do so, it changes both field names to one or the other. It wont allow me to have a phone and fax number. I am using 1.6.2.1.1.

    I'm unable to recreate this issue and it's not being reported by any other users so this could be a site specific issue being caused by other code on your site.

    When I attempt to replicate what you describe I encounter no problems adding multiple Phone Fields to a form and editing each of them individually. See this screenshot which shows the form I created containing 3 Phone Fields with each being named something else:

    What you may want to do is clear your browser cache and then re-edit the form and see if the issue still happens, it could be a browser cache issue.

    If it is not, then it could be caused by code in your theme or another plugin causing issues with the JS used in the form editor. In that situation you'd need to test for conflicts by following the instructions here:
