Possible to do a referral form like this one?

    Hi,

    Just wondering if it's possible to do a referral/rewards form like this one:

    http://www.omargroupcpa.com/CoolRewards.php

    That one is produced by Infusionsoft, so I don't know if Gravity Forms is capable of something similar?

    I don't see a reason why it shouldn't. This just looks like a basic form, just a little more fields :)

    You can even continue to use Infusionsoft with Gravity Forms thanks to this third party add-on - http://wordpress.org/extend/plugins/infusionsoft/
