Problem with Formatting of form with Prophoto3 theme

    Hi
    I have created a contact form for my website but when I embed it in a page the formatting looks strange. I am using the Prophoto3 theme. When is strange is I have friends that have the same theme but are not encountering this problem. The form is here:

    The name fields and address fields are not displaying correctly.
    Any suggestions?

  2. Your default form stylesheet isn't being loaded. That's why the formatting is off.

    Check your settings page and make sure the "Output CSS" option is set to yes.

  3. The Gravity Forms CSS isn't being output, which means the Forms aren't being styled. Other scripts are being output, so it looks like CSS output is turned off. Go to your Gravity Forms Settings page (Forms > Settings) and make sure CSS output is turned on.

    Also if you are running any kind of caching plugin, make sure you clear the cache.
