The name you use for the custom meta doesn't need any special formatting, just simple text. For example, if I want to track someone's favorite color, I can simply call it "color" and point it to the field on my form that has the information.
When you look at the entries for the form, do you see the address information on the entry when you View it?
I do not know what you mean by the "account page", but custom meta fields are not automatically added to WordPress user profile pages.
