Second Page of form not showing...

    Hi there. I have a form all loaded, but the second page of the form doesn't show- tried different browsers- no luck- any suggestions? Form can be found here...

    What am i missing? or forgetting?

    The next button appears for me and I was able to submit the form using Firefox 15 on Windows 7. You should have one fake submission from me with very little information in it.
