The Generated Total for the form is not showing up like $1.00 for example it just shows number. So if the total is ($5.00) then it shows only (5) Please advise of a fix ASAP.

Looks like you have a JS error regarding a plugin called portfolioslideshow: Screenshot
I exported your form and imported it into my dev site and you'll see it works as intended: Screenshot
So your problem most likely lies with that plugin above.

Can you deactivate that plugin to verify the forms correct themselves? Also, make sure you are running the latest version of GF (1.6.4.2) http://www.gravityhelp.com/downloads/

This seems to be a WordPress version issue actually. Is there a reason you haven't updated to the recent release of WP and GF?
