Theme conflict

Form won't show up on my contact page. http://wirkenphoto.dreamhosters.com/site/#/contact/

It will show up if I switch to twenty ten, so I'm assuming it is some kinda theme conflict. I'm hoping someone will give me a clue as to where to start hacking at this. I know enough code to get myself in trouble but not enough to figure it out from scratch.

Figured out that the form shows up if I turn of condition logic.
My css is all messed up but at least the form is there.

Any advice on troubleshooting the JS conflict for the condition logic?

  3. Yep, some scrip in your theme t is breaking the Gravity Forms conditional_logic.js script. That's why the form isn't being displayed properly.

You can re-enable the conditional logic, walk through the themes header.php file, comment out the script references one by one, save and refresh the page to see if the form shows. Once you've found the culprit then we can take a closer look and see what part of the script might be causing the conflict.
