A form with a single field to "search"?
When filling out the field, he tell you if a name already exists or not.
example:
In Form 1 (Registration): In the ID field 1 (name) I registered the name "francisco"
In Form 2 (Search): By filling out the Form 2, Field ID 1 (francisco) = Search button
Result of the form 2: "Name already registered"
(Search) By filling out the Form 2, Field ID 1 (edson) = Search Button
Result of the form 2: "Name registration for free"
