Title / Description Not Working

  1. Hello, I have entered a 'Title' and 'Description' as part of the Form Settings, but they do not show up on my website:
    http://bartlettpairphotography.com/contact/

    They do, however, show up in my preview:
    http://bartlettpairphotography.com/wp-content/plugins/gravityforms/preview.php?id=1

    I did not try to use any custom css scripts...

  2. The form title and description are optional and can be turned on/off.

    Does your shortcode set the title and description values to false?

    [gravityform id=1 title=false description=false]

    If so, you can remove the "title=false description=false" and those elements will be rendered in the page along with your form.
