On a form, even in Preview mode, after clicking submit.

  3. This occurs with 2011 theme and no plugins activated. It also occurs using the preview version of the form inside the Gravity Forms control panel. GF is up to date and we are using Genesis as the framework.

Can I have you remove and re-install a fresh download of Gravity Forms through FTP and see if that resolves the issue?
