Form not displaying in HTTPS

    My form on this page won't appear at the HTTPS address but it will for the HTTP:

    https://www.evgonetwork.com/multi-family-i-own-manage-or-develop-an-apartment-community/
    http://www.evgonetwork.com/multi-family-i-own-manage-or-develop-an-apartment-community/

    It's only occurring for this form and this page and form are no different from others.

    It looks like your secure url page has a JS error on it. See this screenshot.

    Which is coming from the lightbox-plus plugin. I also noticed that your site is running two versions of jQuery. In your head you are running the latest 1.7.2 and then a little further down you are loading 1.5.1. See this screenshot.
